When to use this form
Use these notes during a parent-teacher conference to keep the conversation focused and to record what everyone agreed to. Teachers can fill one out per student, and parents can bring their own copy with questions written in.
Conferences are short, often 15 to 20 minutes. A simple form keeps the time on the child instead of on scribbled notes.

Common mistakes to avoid
- Starting with problems. Open with strengths, so families know you see their child.
- Too many goals. One or two focused goals beat five vague ones.
- No follow-up date. Without one, goals are easy to forget.
- Jargon. Say “reading at a fourth-grade level,” not a test acronym.
