When to use this roster
Coaches and team parents use a roster and contact sheet at the start of every season. It puts every player, parent and coach on one page, so you can share schedule changes, organize carpools and reach a parent fast if something happens at practice.
It works for youth sports, adult rec leagues and school clubs. Print one for the coach’s bag, and share a copy with families if everyone agrees.

Common mistakes to avoid
- Sharing contacts without asking. Get permission from families before sharing the roster with the whole team.
- Missing a second contact. Collect two ways to reach each family.
- Leaving medical info on the shared roster. Keep allergies and medical notes on a separate sheet only coaches see.
- Not updating it. Fix phone numbers and late sign-ups right away.
Frequently asked questions
Should medical information go on the roster?
Not on a copy you share with families. Coaches should keep allergies, medications and emergency medical info on a separate, private sheet.
How many players should a team have?
It depends on the sport and age group. Check your league’s rules for minimums and maximums.
Can I use this for adult teams?
Yes. Leave the parent column blank or use it for an emergency contact.
What’s a team parent?
A volunteer who handles logistics like snacks, carpools and end-of-season celebrations, so the coach can focus on coaching.
